* Court Ordered Custody (if the parent(s) listed on the birth certificate)

* 3 Proofs of Residency
* Proof of Residency must be in parent(s) name
* Post Office Box addresses CANNOT be used as proof of residency
* Proof of Residency must show a 911 Physical Address within Plaquemines Parish
* Proof of Residency must be current
* Driver's License, Car Registration, or Voter Registration WILL NOT be accepted as proof of residency

We accept the following:
* Utility Bill(s): electric, gas, water, cable, or phone bill
* Lease agreement
* Homestead Exemption
